How to refer your patient

Referring a patient is simple.

In Epic, make an Interventional radiology referral online by using the referrals preference list (Epic office access) or by creating an Orders Only encounter (Epic inpatient access), and select the Elmhurst Hospital campus located at Center for Health, 1200 S. York Street, Suite 3100, Elmhurst, IL 60126.
Not on Epic? Refer your patient by calling 331-221-0560 to schedule an initial in-person consultation for your patient.
All consultations take place at the Elmhurst Hospital campus located at the Center for Health, 1200 S. York Street, Suite 3100, Elmhurst, IL 60126.
