Navigating Treatment

Individuality matters. Our Endeavor Health/Linden Oaks Behavioral Health team will collaboratively design your personalized treatment plan with your input. Whether it's addiction or anxiety, achieving life balance requires assistance. To begin our programs, schedule a confidential behavioral health assessment.

How behavioral health works

Our approach is multidisciplinary and personalized. Your care team, consisting of registered nurses, behavioral health professionals, occupational therapists, dietitians, clinical therapists, and psychiatrists, is committed to your complete recovery. We address every aspect of your well-being—biologically, psychologically, socially, and spiritually. To enroll in our inpatient or outpatient services, schedule a confidential behavioral health assessment.

What to expect

Identifying the issue is the first step towards recovery, similar to any other illness. Our confidential behavioral health assessment, conducted by licensed counselors, evaluates your symptoms for a better understanding of your needs. The collected information guides the creation of a customized treatment plan. The assessment, typically lasting one to three hours, includes a discussion with a counselor, medical and psychiatric history review, and insurance details.
For safety, entry into the treatment area requires securing belongings and undergoing a metal detection screening. We appreciate your cooperation in maintaining a safe environment.

Level of care recommendation

Following your behavioral health assessment by a Linden Oaks counselor, they will collaborate with a psychiatrist on the findings. If you've authorized it through a release form, we'll also consult with your physician. We'll then discuss the assessment results with you and your family, provide recommendations for the level of care and program, and guide you on the next steps.

Levels of Care

Linden Oaks staff may suggest adjusting your level of care based on ongoing clinical findings in your treatment. As your levels of care change, your insurance coverage may also be affected. We will verify your benefits at each level of care and inform you of any changes.
