Pre-register for your hospital stay Pre-registration should be completed as early as possible in your pregnancy. Your obstetrician can give you a form to fill out, or you can complete your pre-registration online.

If you choose to fill out the paper form, it should be returned to the Outpatient Registration office in the south lobby of Edward Hospital or mailed to:

You wil also need to include a copy of your photo ID and a copy of your health insurance card (front and back). Please note, you will still be asked to sign a "consent for treatment" form once you arrive at the hospital. We will also ask to see your ID and insurance cards.
Check your insurance coverage The average length of stay for a mom having a vaginal delivery is 2 nights. Check with your insurance company to be sure of your coverage and that both your delivering doctor and your baby's doctor are covered by your insurance plan.
Birth certificate and social security number Your baby's birth certificate form must be completed and signed prior to discharge. Click here for more information on obtaining a copy of your child's birth certificate. Your child's social security card will be mailed to your address approximately 12 weeks after birth.
