Birth certificate: How can I request a copy of my child's birth certificate?

You will be given a form prior to your discharge that you can mail in to request a copy of your baby's birth certificate. After you mail in this form, it may take several weeks to obtain the birth certificate copy.

You may also request a copy of your baby's birth certificate by contacting one of the following:

• For information about a birth certificate request from the Illinois Department of Public Health, click here. This site allows you to submit the request online or by mail for a fee of $15-$43.

• For information about obtaining the birth certificate from the DuPage County Clerk’s office, click here. You can print off the form and mail in your request or take it in person to the DuPage County Clerk's office in Wheaton. If you decide to apply for your copy in person, you will need to bring a photo ID in addition to the other information. The fee for a birth certificate is $10 for the first copy.

• To request a copy of the birth certificate from the DuPage County Health Department, fill out the on-line form: click here. This form also requires you to fax a photo ID. The fee for the first birth certificate is $15.
