EDWARD WALK-IN CLINIC: JUDI BEAUREGARD, APRN, BC

Judi Beauregard, APRN, BC

Judi has been a nurse for more than 30 years. She receiver her Bachelor of Science in Nursing from Montana State University and her Master of Science in Nursing from Northern Illinois University. Judi is a licensed advanced practice nurse and a board-certified family nurse practitioner.

EDWARD WALK-IN CLINIC: TIMOTHY COLLINS, MSN, FNP-BC

Timothy Collins, MSN, FNP-BC 

Tim received his bachelor of science in nursing from Troy State University, Troy Alabama. He continued his studies at Uniformed Services University of Health Sciences in Bethesda Maryland where he received his masters of science in nursing. A board-certified family nurse practitioner, Tim was actively involved in providing health care for the Air Force and received numerous military awards for his work.

EDWARD WALK-IN CLINIC: AMY FROMMELT, FNP-BC

Amy Frommelt, FNP-BC

Amy received her bachelor's degree in nursing science from Northern Illinois University. She continued her studies at Northern Illinois University where she later received her master's degree of science in the family nurse practitioner program. She is board-certified as a family nurse practitioner through the American Nurses Credentialing Center. Amy is also a member of the American Academy of Nurse Practitioners and American Academy of Critical Care Nurses.

Aleta Hawkins, PA-C

Aleta received her Bachelor of Science in Health Science from Benedictine University. She continued her studies at Des Moines University in Iowa where she received her Master of Science in Physician Assistant Studies. A board-certified physician assistant, Aleta is actively involved in community service and has volunteered her time helping underprivileged patients, disabled children, and the elderly.

Terri Kozubik, FNP-BC

Terri received her Bachelor of Science in Nursing from Lewis University, and her Master of Science in Nursing from Rush University. She continued her studies at Northern Illinois University where she received her post-master's degree in the Family Nurse Practitioner Program. A nurse for more than 30 years, Terri is a board-certified family nurse practitioner as well as a board-certified emergency nurse.

Amelia Polheber, FNP-BC

Amelia received her bachelor's degree in nursing with honors from the University of Arizona in 2006. She went on to receive her master's degree in nursing from the University of Arizona in 2009, also with honors. She became board-certified as a family nurse practitioner through the American Nurses Credentialing Center. Amelia is a member of the American Nurses Association and the University of Arizona Honors College Alumni Association.

Anna Risty, FNP-BC

Anna received her bachelor's degree in Nursing from University of Illinois at Chicago in 2000. She went on to receive her master's degree in Nursing from Northern Illinois University and became board-certified as a Family Nurse Practitioner through the American Nurses Credentialing Center. A member of the American Academy of Nurse Practitioners and Illinois Society for Advance Practice Nursing, Anna is fluent in Polish and speaks conversational Spanish.
