Planning Your Visit

To schedule your appointment, please contact Edward Outpatient Rehabilitation at (630) 527-3375 and select "4" at the patient menu. This will direct you to one of our patient services representatives who can schedule an evaluation and any follow-up appointments with the appropriate therapist depending on your individual needs.

What you'll need:

• Your physician's referral
• Insurance authorization (see Insurances Accepted)
• Your insurance card
• A photo ID
• Appropriate dress so that the therapist is able to view and evaluate the area you are being treated for.

Please arrive 20 minutes prior to your appointment time to allow for registration completion, additional medical history, and functional assessment surveys to help us treat you better. This way the therapist can start your evaluation at the scheduled time.
