Providers & Schools

Endeavor Health/Linden Oaks Behavioral Health is your resource for timely information about pressing mental health topics. We offer training programs and professional seminars throughout the year with speakers on the cutting edge of behavioral health treatment. Attendees are able to learn, network and earn professional CEU’s.

Make a referral

At Endeavor Health/Linden Oaks Behavioral Health, we prioritize seamless referrals for patients and providers, emphasizing continuity of care. Referred patients undergo a behavioral health assessment by a licensed professional, who collaborates with the referring provider to determine the most suitable level of care. Walk-in assessments are offered daily at our Naperville hospital, with appointments also available at our outpatient centers in Naperville, Hinsdale, St. Charles, Mokena, Plainfield, and Woodridge.

Continuing education for Mental Health Professionals

Our professional seminars are open to community providers, referral sources and other mental/behavioral health professionals. It is a great opportunity to network with colleagues, gain a fresh perspective and find new ways to help patients. CEU’s are provided and registration is required. View our schedule.

Training and presentations

Linden Oaks staff members include a team of experts in the field of mental health who are available to provide training and presentations for schools or provider offices. Presentations can be created based on the need of school staff, providers, parents, students or patients. Linden Oaks is able to offer continuing education units (CEUs) and for social workers, counselors/therapists, psychologists, and teachers.

In-school services

  • The Crisis Response Team at Linden Oaks is a service designed to help those who may be in need of mental health assistance in the event of a crisis or tragedy.
  • Linden Oaks can provide school liaisons, who will establish a relationship with an adolescent patient’s school to ensure a smooth transition from Linden Oaks back to the school environment. If a release of information permission form has been signed, the liaison will provide constant communication with school representatives.
  • Linden Oaks works closely with schools throughout the academic calendar year by supporting activities in schools that promote positive mental health and self-care. An example: our annual support of Depression Screening Awareness Month.

Provider Services

Linden Oaks can provide assistance to referring providers and schools to ensure a collaborative relationship. Linden Oaks community liaisons are available to set up tours, provider meetings or answer any questions a referring provider or school may have.
