Our certified nurse practitioners provide high quality health care to patients of all ages. They are registered professional nurses who have completed nursing school, advanced graduate education and clinical training. Our nurse practitioners can evaluate your health; diagnose and manage your medical conditions; prescribe medication or treatment; obtain consultation or referrals; and coordinate preventive medicine and screenings.
EDWARD MEDICAL GROUP: JENNIFER CHLUDZINSKI, BSN, RN, APN
Jennifer Chludzinski is a certified Nurse Practitioner who supports preventive health practices and enjoys patient education. She received her Bachelor of Science Degree in Nursing from the Universiy of Illinois at Chicago and her Master of Science Degree in Nursing from Northern Illinois University - both degrees obtained with Honors. Jennifer has a critical care background and has been affiliated with Edward Hospital since 1996.
EDWARD MEDICAL GROUP: SANDY DAVIS, BSN, RN, APN
Ms. Davis is an acute care Nurse Practitioner. She received her Master's degree from Rush University School of Nursing and completed a Post Masters Certificate Program at the University of Illinois at Chicago where she became an acute care nurse practitioner. Board certified in acute care nursing, Ms. Davis has special interests in women's health, preventive medicine and cardiovascular care.
Julee received her bachelor's degree in nursing from Purdue University. She received her master's in nursing from Rush University and became certified as a Family Nurse Practitioner. Julee is a member of the American Association of Nurse Practitioners.
Sandra Mazur is a certified Nurse Practitioner. She received her Bachelor of Science Degree in Nursing and her Family Nurse Practitioner Certificate from Purdue University. Sandra currently lives in Naperville.
Ms. Warnes, Advance Practice Nurse, received her Master's Degree from the University of Illinois at Chicago. She is a certified Family Nurse Practitioner and has continued to practice in the Chicago area since 2003. Kristine's special interests include pediatrics, adolescent health, women's health, weight management and diabetes. Kristine supports preventative health practices with all her patients and looks forward to meeting and caring for you and your family's needs.
