How can I obtain a copy of my child's birth certificate?

If you want a copy of your baby's birth certificate, you may request it from the Illinois Department of Public Health (IDPH), from the DuPage County Clerk of Court’s office, or from the Vital Records Department of the DuPage County Health Department.

• For information about a birth certificate request from the IDPH, click here. This site allows you to submit the request online or by mail for a fee of $15-$43.

• For information about obtaining the birth certificate from the DuPage County Clerk’s office, click here. You can print off the form and mail in your request or take it in person to the DuPage County Clerk's office in Wheaton. If you decide to apply for your copy in person, you will need to bring a photo ID in addition to the other information. The fee for a birth certificate is $10 for the first copy.

• To request a copy of the birth certificate from the DuPage County Health Department, fill out the on-line form: click here. This form also requires you to fax a photo ID. The fee for the first birth certificate is $15.
