Insurance
As an American Diabetes Association Recognized Diabetes Center, most insurances and Medicare will cover diabetes education provided by our staff at the Edward Diabetes Center.
Please call your insurance company before your first visit to clarify your coverage and what your financial responsibility might be. To make things easier for you when calling your insurance company, ask to speak to the chronic disease manager for diabetes. Most insurance plans provide this service.
If you have an HMO, please contact your physician’s office and get a referral for the number of visits that you will need prior to your first appointment. You can call the diabetes center for clarification on how many visits you might require.
Typically, the Step Program for Type 2 diabetes is 4 visits, or a total of 6 hours.
An order from your physician or healthcare provider is needed in order to bill your insurance for your education.
Please call your insurance (new patients and returning patients) to find out what is their preferred blood glucose meter. We can then give you the meter that your insurance prefers so that you will have the lowest co-payment possible when ordering supplies.

Parking
Free valet parking is available if your appointment ends before 6:00 p.m.
We recommend parking in the north parking garage and follow the directions for Medical Office Building #2 (MOB 2), which is actually 120 Spalding Drive. We are located on the second floor in Suite 205.
At your appointment
Please arrive 10 minutes prior to your appointment time to complete a short health history regarding your lifestyle as it pertains to your diabetes or pre-diabetes as well as to sign a consent form for the education that you will be receiving.
Please bring with you a picture identification card, your insurance card, a list of medications you are on and, if you are already testing your blood glucose, a log sheet of your most recent readings.

Call (630) 527-3213 if you need to cancel and reschedule an appointment.
Whenever possible, if you can give 24 hour notice, it would be most appreciated.
