The Edward Cancer Centers in Naperville and Plainfield offer full service out-patient infusions to Edward physicians. We accommodate ambulatory patients Monday through Friday from 8:00 a.m. to 5:00 p.m. We require a valid Edward physician order to schedule an appointment. Please contact your Edward physician for further details.
For Professionals
At the Edward Cancer Centers, we value our relationship with our physicians. We are a full service infusion center set up to meet the needs of your patients. It is important that your patient be independent and ambulatory.
We understand arranging outpatient infusion services for your patients can be time-consuming. So we're making the referral process easier. Simply call (630) 646-6070 and be prepared with the following:
- Patient's full name and date of birth
- Patient contact information
- Current insurance information/Prior authorization, if applicable
- Written order (required) on Edward Cancer Centers Order Forms
- Valid diagnosis code
- Current history and physical
- Any recent labs/tests, if applicable
We will then ask you to send a completed Edward Cancer Centers Order Form to our fax (630) 548-6617. Once your order is approved, we will contact your patient to arrange an appointment.
