Linden Oaks values our relationship with professionals in the medical, therapeutic and educational fields. And we understand that making a referral can sometimes be time-consuming. So we're making the referral process easier. Simply call (630) 305-5027 and be prepared with the following:
- Patient's full name and correct spelling
- Date of birth
- Reason for the referral
- What insurance the patient has so that we may identify whether we are within their network of coverage
After we receive your referral, your patient may call us to schedule an assessment, or simply walk in at any time and be triaged, as identified through the referral you made for them. As a walk in, your patient may need to wait to be seen, but will be seen that day.
We will assess your patient at that time and our assessment will determine the level of care needed. Your patient will be accepted through inpatient, partial hospitalization, outpatient care, or we will make a referral to a more appropriate resource.
We will provide your patient with a release form allowing us to contact your office and indicate the status of your patient, should he or she chose to sign it.

The Linden Oaks treatment team of psychiatrists, psychologists, social workers/therapists, registered nurses, dietitians, occupational therapists, art therapists, medical doctors, and hospital chaplains provides innovative care with dignity, compassion and respect for each individual.
